First Visit
Your initial appointment will consist of an initial detailed examination explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the initial exam. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment and /or hygiene services on another day. New patients usually have a two appointment process for examination and hygiene services unless emergency care is necessary.
Consultation Visit
If you would prefer to visit the office before scheduling, we invite you for a tour and no fee consultation to meet the Doctor and Staff.
Please assist us by providing the following information at the time of your examination:
- Your referral slip, dental history, and any X-rays if applicable.
- A list of medications you are presently taking.
- If you have medical or dental insurance, bring the necessary information, insurance card, and booklet if possible. This will save you time and allow us to help you process any claims.
IMPORTANT We see many children for dental care in our practice. A parent or guardian must accompany all patients under 18 at the examination visit.
Please alert the office if you have a medical condition that may be of concern prior to your care (i.e. uncontrolled diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e. antibiotics for pre-med).
X-Rays
If your previous dentist has taken recent x-rays (within 36 months), you may request that they forward them along with a copy of your treatment record and periodontal charting to our office. We are happy to assist you in gathering previous records. If additional films are necessary, they can be taken at our office.
